Short answer
The door: if its construction does not communicate how it is used, the error is programmed into it and will recur for most users.
1. Position
Norman works as a cognitive scientist and design theorist. His contribution consists in carrying concepts from the psychology of perception into an applied discipline.
2. Error as defect
The central claim: if users systematically make the same mistake, the cause lies in the object. This moves blame from person to construction and makes error measurable.
3. Affordance
The key concept describes properties of an object that suggest a possible way of handling it. A flat plate implies pushing, a handle pulling; a conflict between these signals produces error.
4. A refinement of the concept
He later clarified that perceived rather than physical affordance matters more for design. What counts is not what an object permits but what a user believes possible.
5. The signifier
To remove confusion he introduced a separate concept for explicit cues: labels, arrows, markers. An affordance exists objectively; a signifier communicates it.
6. Mapping
The relation between the arrangement of controls and of controlled objects determines the number of errors. Hob burners and their knobs are the canonical example of poor mapping.
7. Feedback
A system must report that an action has registered. An absent response produces repeated pressing and a subsequent error — a described and reproducible mechanism.

Construction can physically preclude an incorrect action: a connector that fits one way only removes the question of instruction. This is the most reliable form of design protection.

A user builds an idea of how a device works and acts according to it. The designer’s task is to make that idea coincide with the actual mechanism.

He himself noted that human-centredness applied literally leads to improving the existing rather than reconsidering the task. That is an honest limitation of the method.

In later work he added that objects are judged by more than efficiency: attractiveness affects tolerance of difficulty. This refines rather than cancels the earlier claim.
